PS: I have additional documents to support this project if neededThe defined problem is as follows:I interviewed a couple of ALS patients/residents and the nursing supervisor on a vented floor in a skilled nursing home. These patients are at the end of the
disease process, they are not able to communicate verbally, not able to take
anything PO, not able to move at all, and not even able breathe
without the ventilators. It seems like a group of patients who need a lot of
attention and skilled care. The facility is very short-staffed first of all,
and they are not training their staff the proper way. These patients rely
heavily on their Tobii to communicate and barely one or two floor staff know
the basic setup for Tobii at all. Whenever there is a technical issue going on
with the Tobii, the patient is completely helpless. They do have a speech therapy
team who are able to help patients to set up Tobii, but most of the time no one
is answering the phone when a nurse is trying to report a problem. It takes a couple or several days for someone to fix an issue related to Tobii. This means
that the patient was not able to communicate his needs at all for several days!There are not enough CNAs or nurses on the floor. The ratio is 1 nurse: 7 vented patients! And usually 2-3 CNAs for 10 patients depending if
people call out or not. Again, these patients are completely dependent. Each
patient needs at least 2 assistants at all times and also they can not verbally
communicate. It takes a very long time for these patients to type up their
needs/questions with their eye movements. It is very time-consuming for the
staff, and frustrating for the patients when they know everyone is so busy.Most of the staff working there are overworked and are not happy
with the management. Sometimes they cannot help but show it on their
faces, especially if a patient needs some extra attention that day. It makes
the patients feel like they are asking for too much when they only asking to be
repositioned every so often because they are in constant pain due to their
disease process.Preparation
In this assessment, you’ll develop an intervention as a solution to the health problem you’ve defined. To
prepare for the assessment, think about an appropriate intervention, based on your work in the
preceding assessments, that will produce tangible, measurable results for the patient, family, or group.
In addition, you might consider using a root cause analysis to explore the underlying reasons for a
problem and as the basis for developing and implementing an action plan to address the problem. Some
appropriate interventions include the following:

Creating an educational brochure.

Producing an educational voice-over PowerPoint presentation (narrator’s script needed).

Creating a teaching plan for your patient, family, or group.

Recommending work process or workflow changes addressing your topic.
In addition, you may wish to complete the following:
